Definition

AKI is defined as any of the following;

  1. Increase in SCr by ≥ 0.3 mg/dl (≥ 26.5 µmol/l) within 48 hr; or
  2. Increase in SCr to ≥ 1.5 times baseline, which is known or presumed to have occurred within the prior 7 days; or
  3. Urine volume <0.5 ml/kg/hr for 6 hr
  4. Small fluctuations in serum creatinine have been included in the spectrum of AKI as they have an adverse effect on the overall outcome of the patient.

Discussion

  1. Acute kidney injury (AKI) is associated with increased morbidity and mortality in patients. The risk increased if associated with other co morbidities. AKI is also an independent risk factor for chronic kidney disease.
  2. At Kauvery hospital, Cantonment we analysed the clinical outcome of AKI data for patients admitted between January 2023 to October 23.
  3. A 394 patients were included in the study.249 were male patients and 145 were female patients. Most of our patients were aged 50 and above and many of them had co morbidities like DM, HT, CAD, CLD, CKD. The most common cause of AKI in our study was sepsis (24.2%) followed by respiratory tract infections (19.9%). The next common etiologies were Poisoning (7.4%) and Dengue (6.9%).7% of patients with AKI had a pre-existing CKD. Presence of pre-existing CLD, CAD, and COPD were significant risk factors for developing AKI.
  4. The overall mortality in our study was 21.8%. The 58.3% of patients were discharged alive with improving renal parameters .outcomes and the remaining 19.5% people could not be assessed as they were discharged due to various reasons.

Conclusion

AKI is an important risk factor for increased morbidity and mortality in ICU setup in any tertiary care hospital. Presence of AKI in association with co morbidities increase the patient’s mortality. Infections are the leading cause of AKI in our hospital. Identifying AKI early and timely appropriate treatment will reduce the burden on both patient and hospital
