Online Consult Patient Telemedicine Consent & Disclaimer
I hereby attest that I have requested and authorized The Treating Doctor / Kauvery Hospital for an online consult for me or for the person requested regarding the diagnosis mentioned in the conditions section.
The Treating Doctor will reach his or her conclusions regarding my medical diagnosis based solely on the information provided by me.
I understand and agree that:
Online consultation is solely based on the information provided by me and, in the absence of a physical evaluation, the Kauvery Hospital / treating doctor may not be aware of certain facts that may limit or affect his or her assessment or diagnosis of my condition and recommended treatment.
Online consultation is very different from a regular face-to-face examination and the Treating Doctor providing the consult is limited by the written information and imaging provided by me. Accordingly, the diagnosis I will receive is limited and provisional.
An online consult is not intended to replace a full medical face-to-face evaluation by a doctor. The Treating Doctor is only rendering an online consult and does not assume any responsibility for my continued medical care or treatment.
My medical information will be handled with strict confidentiality, privacy, and security; however, I understand there are risks associated with any electronic transfer process from one location to another.

I solely assume the risk of the limitations set forth herein, and I further understand that no warranty or guarantee has been made to me concerning any particular result related to my condition or diagnosis.
Communication Consent:
I declare that by agreeing to the Terms and Conditions set forth herein, I give permission to Kauvery Hospital to reach out to me through digital modes of communication,
Including but not limited to WhatsApp, SMS, and e-mail. The communications may be pertaining to updates regarding my appointment, reminders for services I have subscribed to, delivery of results for the service taken at the hospital, any offers or promotions run by the hospital, and other such communication as deemed necessary by the hospital for seamless delivery of service.
Disclaimer and Release
I hereby completely and irrevocably release Kauvery Hospitals and its parent and sister corporations and their respective medical staff members, doctors, consultants and other health care professionals, insurance providers, administrators, officers, employees and directors (collectively, the “Kauvery Parties”) of any and all errors and omissions, known or unknown, foreseen or unforeseen, knowingly or unknowingly, as well as all claims, actions or damages arising from or in connection with the online consult, conclusions or recommendations provided by Kauvery Parties or its doctors. Furthermore, I agree that the Kauvery Parties have no liability or responsibility for the accuracy or completeness of the medical information submitted to them or for any errors in its electronic transmission.
As a condition to receiving the online consult service, I have read and acknowledge that I have given this consent of my own free will.
By accepting and agreeing to these terms, I acknowledge and agree to assume the risks of the limitations set forth herein.
Booking Appointments
While Kauvery Hospital will try to ensure a confirmed appointment with a doctor for a patient/or a user who requested an appointment on the Website or App, Kauvery Hospital does not guarantee that the patient/ registered user will get a confirmed appointment. Further, Kauvery Hospital or its doctors have no liability if such an appointment is confirmed but later canceled, postponed, or rescheduled by the doctor, or if the medical service provider is not available as per the given appointment time.
Cancellation
Consultations can be booked, rescheduled, or canceled free of cost up to 60 minutes prior to the start of the appointment for an audio/video consultation (“Online Medical Consultation”) or physical consultation. You are allowed to reschedule an Online Consultation up to a maximum of three times and are allowed to reschedule a physical consultation once.
Payments for Online Medical Consultation or physical consultation made directly on the Platform will be processed as provided in the ‘Refunds’ section below.
Payment & Refund
You may use any payment service, like UPI, internet banking, or debit or credit cards for making payments for availing any of the Services over the Platform (“Fees”). You shall be directed to our third-party payment aggregator/gateway (“Payment Aggregator(s)”) once you proceed to make payment of the Fees. Once re-directed to the Payment Aggregator, you agree to be bound by the terms and conditions and privacy policy prescribed by the third-party payment gateway/aggregation partner, the basis on which, the transaction shall take place. We will share your details with the Payment Aggregator as per the terms of our privacy policy.
Refunds for all eligible cancellations are issued through the payment method used at the time of purchase, except for cash payments made under the Pay on Delivery mode of payment.
Refunds will only be made against genuine and trackable cancellation requests generated by you by clicking on the cancellation button provided in the App’s user dashboard in your Kauvery Kare account. Refund requests against delays, the nature of advice obtained, the efficacy of treatment, and the health outcomes of the consultation will not be entertained.
Refunds may be processed within 15 working days from the receipt of a request from you. All communications with regard to refunds should be sent to the helpdesk.
