A 70 years male came with abdominal pain after food intake & loss of weight for 3 months.

Endoscopy revealed a submucosal lesion with central ulceration in the duodenal bulb. A biopsy done showed a Neuro Endocrine Tumour.

Endoscopic submucosal dissection was done

Resected specimen

Biopsy report:

Gross: Specimen measures 2.3 x 1.8 x 1.2 cm. A central elevated lesion measuring 2.1 x 1.8 x 1.1 cm is seen with a firm grey white cut surface. Clearance is 0.2 cm from the peripheral resected margin and 0.2 cm from the deep resected margin.

Microscopic description:

1) Peripheral resected margin (shaved), free of tumour.

2) Lesion: Sections show duodenal wall with submucosal tumour arranged in organoid pattern and as trabeculae composed of polygonal cells with moderate eosinophilic cytoplasm, hyperchromatic round nuclei and inconspicuous nucleoli. Mitotic activity is 0-1/10 high power fields.

Impression: Endoscopic resection of duodenal neuroendocrine tumour. Suggestive of Well Differentiated Neuroendocrine Tumour, Grade I. Peripheral and deep resected margins are free of tumour.
