What is your diagnosis?

Answer: Palmar hyper linearity

Hyper linearity is a thickening of the skin on the palms and soles with an increase in the number of lines in the skin.

Palmar hyper linearity is a typical clinical feature of Filaggrin gene (FLG) null mutations. There are reports of FLG mutations and allergic sensitization; however, reports on the relationship between palmar hyper linearity to sensitization are limited.

This is a monogenic skin disorder is caused by Filaggrin deficiency, a genetic mutation that is present in 8-10 percent of the population. These people’s skin does not retain water well, which leads to dryness and problems with eczema. When the skin lacks this protein, its ability to shield against external dirt and substances is reduced.

Filaggrin deficiency leads to a “leaky” skin barrier that allows higher than normal water loss (explaining the dry, scaly skin), as well as allowing entry of allergens through the epidermis where they trigger inflammatory and allergic immune responses (atopic eczema and allergies).
